Prior to your first appointment, please bring information from your medical provider, such as a prescription or imaging results, and also bring your insurance information. We have paper work for you to fill out, that can be filled out in the office, or downloaded from our website, here.
If you are planning an elective orthopaedic surgery, your physician will likely want you to have postoperative physical therapy. Please set up an appointment prior to your surgery so you can start when appropriate after surgery.
